Program provides home visits to individuals w/developmental disabilities that occurred before the age of 22 yrs. Services can continue throughout a person's lifetime if needed. A nurse visits the home to do an intake and initial assessment to determine the needs and appropriate referrals for services. Assistance with follow-up and continued home visits are made on an as needed basis. For children under 21 call Special Kids Network (Phone:1-800-986-4550).
Family law service provider rendering legal services in guardianship, adoption, custody and estate planning and administration matters offering reduced, flat fee charges for preparation of a temporary guardianship designation.
Provides family- driven and confidential support services to adoptive, kinship, and custodial families. We offer 3 types of services: case management, support groups, and respite care. All services are free and fully funded through the PA State SWAN department. Case management services allow our caseworkers to connect the family with local resources such as: mental health needs, educational services, and birth parent conflict resolutions, along with several other needs and services identified between the family and caseworker. We offer several types of Support Groups within the Erie and Crawford County areas as well as Online Support Groups. The respite support service allows us to stand out amongst other agencies within the state due to our ability to provide the family with $900 in respite funding which can be used for activities such as family memberships to the YMCA, zoos, museums, dance classes, and other extra-curricular activities. These funds can also be used to pay for approved respite care providers.
